I understand that in order to complete the processing of my application, a visit to my home will be scheduled by a representative of Fetch Animal Rescue. By submitting this application, I agree to such a scheduled visit.

I understand and agree that if I rent, my landlord may be asked for a signed acknowledgement or personal meeting, prior to finalizing my fostering for Fetch Animal Rescue.

I certify that all the information provided by me in this application is true and correct.

I give FETCH Animal Rescue permission to verify any and all statements made in this application.

I understand that this application does not guarantee acceptance as a FETCH Animal Rescue Foster.

I accept and agree that I am not financially compensated by Fetch Animal Rescue for fostering a cat for them.

I understand that Fetch Animal Rescue will cover all necessary medical expenses for the foster pet while in foster care.

I understand that Fetch will provide a collar and tag for each foster cat that comes into my care, in addition to ongoing food and litter.

I acknowledge and understand that there are multiple steps in the process to be approved as a foster caregiver for Fetch Animals Rescue. The 4 steps are as follows:

Step 1: Submit an application to foster.
Step 2: Have a phone meeting to go over your application and any additional questions I or a Fetch representative may have. If both Fetch and I agree to proceed we will move onto the next step.
Step 3: Schedule and have a home check visit. If successful and satisfactory to Fetch, I will move onto the final step of the process.
Step 4: The final step is to sign a Cat Fostering Agreement and Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement. Once these are signed off on by me and the Fetch Foster Coordinator, I am ready to begin fostering.

I accept and agree that I can be denied from being a foster caregiver for Fetch Animal Rescue for any reason(s) they see fit. Due to this, I accept their decision fully with no reservations or concerns. Therefore, I hereby agree to indemnify and save and hold harmless Fetch Animal Rescue, its employees, founders, volunteers, agents, members, directors, officers, or any affiliated entities thereof, for their reasons and decision.

I accept and understand that I will be required to sign a Cat Fostering Agreement and Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement in order to foster a cat for Fetch Animal Rescue.

I understand and agree that I am responsible and liable for the safety of each foster cat that comes into my care, for the safety of other animals and people the cat comes into contact with.

I hereby agree to indemnify and save and hold harmless Fetch Animal Rescue, its employees, founders, volunteers, agents, members, directors, officers, or any affiliated entities thereof, from ANY LOSS, LIABILITY, DAMAGE, OR COST INCLUDING BODILY INJURY OR POPERTY DAMAGE ARISING OUT OF OR RELATED TO THE FOSTERING OF AN ANIMAL FROM FETCH ANIMAL RESCUE.

I accept that by completing and submitting this application form, I fully understand and agree to assume all risks involved in any and all duties that I perform in a fostering capacity on behalf of or in connection with FETCH Animal Rescue. I agree to hold FETCH Animal Rescue harmless for any injury(s), loss or damages which I might sustain during the course of my fostering. This waiver does include myself, all of my family members and descendants forever from seeking any legal action whatsoever against FETCH Animal Rescue or its successor organizations or any representatives thereof.

I accept and understand that by clicking the submit button, this will serve as my signature with the submission date as the signature date, and that this application will be printed out by Fetch with my answers, and a formal signature will be required and obtained from me during the home check visit, which is Step 3.
